Registration Procedures

- Registration form and payment must be received by midnight July 7th, 2009.
|
- No refunds after July 3rd. Refunds prior to July 3th, 10% will be with held from refunds.
|
- Use the online registration LINK below to register your team.

Day of Tournament
|
- Finalized Team Entry Form with Tournament Waiver signed by each parent (see form below)
- Proof of age (copy of birth certificate, player pass, etc)
- Medical Release Forms for each player (see form below)
- Teams must check in (with documents) 1 hour before their first game.

Tee Shirts
|
|
|
|
Every player on each team will receive a tournament tee shirt. Each team in each bracket will get different colored tee shirts that can be worn as a team shirt for the
tournament. A sample tee shirt is shown below. We reserve the right to choose the colors and/or change the design of the tee shirts prior to the tournament. Sizes should be
specified on the team entry form.
